Replace Sky lnb with quad?

If I replace the twin Sky HD lnb with a quad lnb (which one?), can I run the 2 original cables to the SkyHD box and the spares to 2 freesat TVs?

Yes.

Are you sure its a twin LNB though and not a quad with two ports unused?

As Sky haven't used/supplied twin LNBs for years as they were unreliable and there was little difference in cost between twin and quads.
